Help Needed: Which Companies have Equity Choice or "Cafeteria" Programs?

I am looking for companies who have "Equity Choice" or "Cafeteria" style programs wherein participants can choose between whether they receive stock options and RSUs, or some other compensation instrument.
If your company has a programs like this, I would very much like t speak with you. If you know of a company who has a plan like this, please list them here, or shoot me an email with the company name (and a contact, if possible).
When I last checked there seemed to be less than 50 companies who truly had plans like this, but I love to chat with those that do.
A few companies who I believe have, or had, programs like this include:
- Citigroup
- Revlon
- Johnson & Johnson
- AON
- Qualcomm?

I know that at one time JPMorgan has this type of program, but am not sure if it still exists today.

I know Pepsi had this kind of plan in place at one time - I don't know if they still do.

Best Buy is "best" example of broader program I know about. At NASPP conference a few years back there was presentation on it. I also recall discussion of plan at Applebee's at that session, but don't know what happened after change of control at the company.
Netflix definitely has one and allowed employees to take 100% of your compensation in stock options (or cash, or restricted stock). I think it eliminated the all option choice year or so before the stock went through the roof! Interesting to think about whether that was good move or not for them.
More companies have it then you may realize at the very top level where they may formally or informally give the very top executive a "choice" of what they want, such as options or restricted stock units. I've noticed this in a few proxy statements, such as Athena Health (fair disclosure: I own Athena stock, although regret not buying Netflix when the market crashed).
